Awake In America's xPAP Donation and Relief Program Shipping Form is a three-section form. One section is a mailing label, pre-addressed to Awake In America; a second section is a receipt for mailing costs, which may be deductible as part of your donation; and the third section is an inventory form to be placed inside the box prior to shipping. The inventory form allows us to confirm all equipment is received, as well as match up all equipment donations to proper donors when preparing donor receipts for tax donation.
